Board Member Application

Please place my name on the nominating ballot for the Board of Directors. By signing the form, I certify that I qualify to be a board member pursuant to the Community’s governing documents.




Provide a 200 word or less informational biography about yourself and why the Community will benefit from your service on the board (previous experience, etc.). The information you provide below will be included verbatim into the candidate biography that will be provided to the Members.

If you are elected to serve on the Association’s Board of Directors, please be advised that the Corporate Transparency Act (“CTA”), a new Federal Law, will require that each Board member submit certain information to the Federal Government, which includes the following:

  • Full legal name
  • Date of birth
  • Current residential street address
  • Unique identifying number from a non-expired US Passport or Driver’s License

Failure to timely submit this information could lead to severe criminal and civil penalties imposed on the Director. Accordingly, please make sure that you are willing to submit this information if you are elected to serve on the Board of Directors for the Association.

The Board reserves the discretion to ask a candidate to resubmit their biography if defamatory references are made to other persons within the Association.
